Parent Chaperone Order Form

I have finally been able to put together the chaperone shirt order form for the new chaperone shirts.  We were finally able to find a gold shirt with white stripes down the side similar to what they were originally.  I have attached the order form for you.  I would like to get these ordered by next Friday September 2nd so I have set the deadline as Thursday, September 1st.  As you will see on the order form, the cost is $39 and you may put whatever you want on the 3 lines on the front of the shirt.  This is the price given to us by the embroidery folks and we are not profiting from this at all.  The material is the same material as the student shirts.  The ladies sizes are form fitting and fit a little smaller than normal polo shirts.  The Men's (Adult) size fit like a normal polo shirt.  Again, both the money and the form are due to me by Thursday, September 1st.  All checks need to be made payable to MHSBPA.
